How the Penis Filler Procedure Works

Up to 2 inches of girth, flaccid and erect, and up to 0.75 inch of flaccid length, typically in one visit of under an hour. After a local anesthetic, the doctor usually places the full volume at a single sitting through a blunt cannula, the technique behind more than 3,000 procedures by doctors affiliated with Upsize™. Most men are back at work the same day.

What It Changes

Filler adds up to 2 inches of girth, both flaccid and erect. How much you gain depends on the volume you choose and the size you start from. It also adds up to 0.75 inch of flaccid length, because the penis retracts less.

Typical results — yours depend on your starting size and the volume you choose, and your doctor tells you what to expect for you at your consultation.

A larger penis gains slightly less from the same volume, since the filler spreads over more surface. That's why the doctor recommends a volume only after examining you.

Erections, sensation and urination are unchanged.

The Procedure, Step by Step

  1. The information session. A patient coordinator talks through what you want, then books you at your nearest clinic.
  2. The examination. A typical visit: the doctor examines you, reviews your medical history and recommends a volume to match your goals. Many men go ahead with the procedure that same day.
  3. Numbing. The doctor numbs the area with a local anesthetic block. You stay awake throughout, with no general anesthesia and no sedation.
  4. The filler. Working through a blunt cannula, the doctor places the filler evenly along the shaft.
  5. Shaping. Before you leave, the doctor checks the result for symmetry and shapes it.

Why This Technique

The blunt cannula does two jobs. It spreads the filler evenly along the shaft, and it usually lets the full volume go in at one sitting instead of across several appointments. This is typical; your provider evaluates what is right for you, case by case, at your appointment.

Nothing is locked in, either. If needed, the filler can be dissolved with an enzyme called hyaluronidase. Full list of risks and complications →

The Filler

The filler is a hyaluronic acid gel2.

Hyaluronic acid is a substance your body already produces. Studies of penile girth enhancement with it report lasting results1.

Recovery and Results

Most men return to work the same day, and there's nothing to wear afterward: no sleeves, wraps or pumps. You'll take a short course of antibiotics, so let the doctor know about any drug allergies. Wait one week before intercourse, so the filler settles where it was placed. A follow-up visit is available at no extra charge if you want or need one. This is typical; your provider evaluates what is right for you, case by case, at your appointment.

The result is usually visible immediately. For the first few days, swelling makes it look larger than it will be. As that settles over two to three weeks, you keep about 90 to 95 percent of the size you saw on day one.

On average, the result lasts about two years.

What Men Say About the Procedure

I saw Dr. Rosenberg at the Upsize location in Beverly Hills and am really happy with the results. The whole thing took about an hour and was relatively painless. There was a bit of swelling after for about a week but it really didn’t stop me from doing any activities or anything. I highly recommend them if you’re thinking about doing this.

I had the procedure done by Dr. Jack D. Friedlander at his Roseville office. He’s a total professional. Explained everything, the procedure went flawlessly, and he even gave me his personal number in case I had any questions. Needless to say I am more than pleased with the results. Totally natural look and feel. Cannot recommend highly enough.

Who It's For

Healthy adult men who want more girth, from their twenties through their seventies, whether circumcised or uncircumcised.

If you'd also like firmer erections, Firm-X™ can be done at the same visit. If you want a fuller head of the penis, glans filler is an option, decided once the shaft is done.

Safety

Serious problems are rare. In a randomized trial of hyaluronic acid penile filler, side effects were mild and short-lived3. A 2026 review of 23 studies covering 2,620 men found that hyaluronic acid had the lowest complication rate of any enlargement method4. One of the ways hyaluronic acid filler differs from silicone and permanent fillers is that it can be dissolved with hyaluronidase.

Risks and side effects

Common and short-lived: swelling, bruising or mild tenderness for a few days to a week.

Less common: infection, which the antibiotics are there to prevent; filler moving from where it was placed; small firm lumps or unevenness, which can be smoothed or dissolved; and, in uncircumcised men, swelling of the foreskin that makes it hard to pull back5.

Rare: an allergic reaction; filler blocking a blood vessel, which needs treatment the same day.

Extremely rare: filler reaching the lungs (pulmonary embolism) — see all the risks →

Call the clinic straight away if pain gets worse instead of better, the skin turns pale or dusky, or you notice numbness, spreading redness or a fever. If you can't reach the clinic, go to an emergency room. Trouble breathing or chest pain means calling 911 straight away.

These fillers are FDA-approved for cosmetic use in the face. Their use in the penis is off-label, which means the FDA has not reviewed that use.

Contraindications

FDA labeling for hyaluronic acid filler lists these reasons not to be treated2, 6:

  • Severe allergies with a history of anaphylaxis, multiple severe allergies, strong reactions to common allergens (atopy), or planned allergy desensitization shots.
  • An allergy to hyaluronic acid products, Streptococcal (Gram-positive bacterial) proteins, natural rubber latex, or local anesthetics.
  • A bleeding or clotting disorder, or a connective tissue disorder such as lupus.
  • Blood thinners or antiplatelet medication. Mention these on your information call, as you may need to wait.
  • An infection, inflamed skin, a sore or an unhealed wound in the area.
  • Scars in the area, or a history of raised (hypertrophic) or keloid scars.
Who should not have it
  • Men who want more erect length. Filler can't add it.
  • Men whose concern is firmness rather than size. Firm-X™ is the treatment for that.
  • Men who want a permanent change. The result lasts about two years on average.

Questions About the Procedure

Does it hurt?

Most men feel pressure, not pain. Typically, before anything else happens, the area is numbed with a local anesthetic block. Some men have mild tenderness or bruising afterward, which usually fades within a few days to a week.

Will I need more than one visit?

Usually not. The full volume you choose typically goes in at one sitting. Many men do come back for more at some point, because they like the result. Filler added later is $660 per ml, regardless of which package you started with. This is typical; your provider evaluates what is right for you, case by case, at your appointment.

Is there a longer-lasting option?

Yes, for men who have had filler before. Some offices offer Radiesse®, a collagen-stimulating filler, blended with the hyaluronic acid so the result lasts longer. It adds $100 per ml.

Like hyaluronic acid, Radiesse® is FDA-approved for the face and used off-label in the penis. Radiesse® is calcium hydroxylapatite, not hyaluronic acid; it lasts longer, and hyaluronidase does not dissolve it — only the hyaluronic acid it is blended with can be dissolved. The rest wears off on its own.
